If you want him to show up, you need to beat the boss without taking a single hit. When he does appear, take Extra Life every time, and treat Through the Wall and Water Walking as easy yes answers too.

The Angel

The Angel gives you a choice between a small ability or a heal. Take the ability in most situations, because those little bonuses stack across a long run in a way one heal never will.
